Plan with Care’s team of inhouse specialists provide wrap-around Care and Wellbeing Services for older people in the South East of England (Kent, Sussex, Greater London and parts of Surrey).
It can be tricky to know what to do when an older person starts to struggle with day-to-day life.
Our Care and Wellbeing Consultants can help – they are experts in wellbeing and maximising the quality of life of older people.
Your Consultant is supported by our inhouse team of specialists in nursing, dementia, social work, capacity assessments, safeguarding, physiotherapy, and person-centred care planning and care management.
First, you’re not sure if the older person you’re looking out for is doing quite as well as they could be. There are signs of short-term memory loss, and you worry that they’re not socialising quite as much as they used to. The odd time they do venture into town they don’t seem quite as confident as they used to be.
When you next spend some proper time with the person, you realise that actually they are having some serious issues getting around and with their hearing. They’re putting a brave face on it - 'I’m fine' or worst case they might quip 'it’s no fun getting old you know!'
It can be tricky to know how to approach these things, but that’s what our Care and Wellbeing Consultants are expert in. Our Consultants know exactly how to adjust the tone, pace and style of a conversation to match the needs of the person they’re meeting in order to maximise their quality of life.
The Plan with Care team has it all; specialists in nursing, dementia, social work, capacity assessments, safeguarding, physiotherapy, and person-centred care planning and care management! We also have a trusted network of solicitors, financial advisors, hearing specialists, Occupational Therapists and Dementia Care Practitioners that we work closely with.
We ensure the basics, enhance quality of life, and exceed expectations where possible.
Between us we’ve worked in the community, in care homes, in Social Services and in hospitals. You’ll only ever have one call to make, to your named Consultant who you can turn to at any time. They have our wider wellbeing community for support and advice to ensure you’re provided with the ongoing expertise and support to thrive and have the best possible quality of life. This can include care at home and live-in care if needed.
